Federal law requires that you fill out a privacy waiver form in order for our staff to assist you. Please print and fill out the form and return it to the district office. The form can be found by clicking this link.
More than likely, your case will be processed in the district office nearest to you. You should send the form to the appropriate district office so that your request can be processed as quickly as possible. The district offices are your best resource for handling casework.
